The providers at StoryArc Therapy are private-pay only and do not accept insurance, however, upon request we are able to provide a “Superbill” receipt for services that you can submit to your insurance provider to seek reimbursement for counseling.

Your card on file will be charged for the full rate of your therapy session at midnight on the day we meet. Once services are paid and your Superbill is issued to you (monthly upon request), you may begin the process of seeking reimbursement for mental health services. Below is a guideline.

How to Submit a Superbill to Your Insurance Company

    1.    Review Your Insurance Policy

    •    Check if your plan offers out-of-network reimbursement for mental health services.

    •    Look for specific requirements, such as claim submission deadlines or additional documentation.

    2.    Gather Necessary Documents

    •    Ensure you have a copy of your superbill from StoryArc Therapy.

    •    Obtain any forms your insurance provider requires for submitting claims.

    3.    Fill Out Required Forms

    •    Complete your insurance company’s claim form.

    •    Include accurate information about your policy and the services you received.

    4.    Attach the Superbill

    •    Include the superbill with your claim form. Double-check that it is clear, complete, and legible.

    5.    Submit Your Claim

    •    Submit the claim and superbill to your insurance company via their preferred method (mail, fax, or online portal).

    •    Keep copies of everything for your records.

    6.    Follow Up as Needed

    •    Contact your insurance company to confirm they’ve received your claim.

    •    Ask about processing times and address any requests for additional information.

    7.    Receive Reimbursement

    •    Once processed, your insurance company will reimburse you directly according to your plan’s coverage.
